Welcome to the Utah State University Food Safety Managers Certificate (FSMC) course. This course covers the learning objectives required by the State of Utah Food Code and provides the exam required for certification. Si desea inscribirse en la versión en Español de este curso, haga click aquí.
The materials are divided into four Modules:
- Module 1: Food Borne Illness
- Module 2: Food Safety Risk Factors
- Module 3: Food Safety Precautions
- Module 4: Food Safety Programs
Each module features five topic-based instructional videos and five Topic Quizzes. There is one Final Certification Exam with 60 questions.
Taking the Exam
The exam can only be taken at an approved USU Testing Center where it can be proctored. You may take the exam up to 3 times. Each attempt has a 24 hr. waiting period before trying again. The USU Testing Services Page has contact information for your local USU Testing Center to schedule an exam. If you cannot reach a USU Testing Center to take your exam, other arrangements may be possible. There is a proctor fee for each attempt due at the testing center when you take your exam. Payment methods accepted at the testing centers may vary. The Food Safety Managers Certificate exams are bilingual (English-Spanish).
Utah FSMC provides the restaurant and food service industry with an up-to-date, comprehensive food safety training and certification program. The course and exam were reviewed in January 2023 and are in agreement with the 2013 US FDA model Food Code, as adopted under Utah Food Code with amendments. All materials were approved by the Utah Department of Health and Human Services and were written to meet the requirements of the Utah Food Safety Manager Rule. Certification is recognized in all counties of Utah. Certification is NOT recognized outside of Utah.
